As we are a medical facility, we must ensure the safety of all patients and staff members. All patients over the age of 4 are encouraged to wear a mask when coming into the clinic. However, following recent Hamilton Health Sciences guidance as of April 2023 masks will no longer be mandatory. The COVID-19 screening protocols continue at Waterdown Family Medicine Clinic.
Please provide your preferred phone number at the time of booking. Make sure that this number accepts calls from blocked callers and answer the phone when the doctor calls. A 1-hour window will be given. Our phone lines are open Monday to Friday 8:30AM-12:30PM and 2-4PM, as well as Saturday 9-11:30AM.
If you do not cancel or reschedule your appointment with at least 24 business hours notice, you may be subject to a $50 “no-show” fee.
Please make an appointment for prescription renewals, or you may be subject to a $10 fee. We prefer not to encourage faxed renewals from pharmacies, as they are time consuming for staff and physicians. We will make every attempt to provide repeat medications at the time of your appointment. However, we do offer a yearly Prescription Renewal Program. Ask for details.
Only email our office if we have requested you to do so for follow-up of a medical issue, or for administrative purposes. New medical requests by email, including prescription renewals, will not be addressed. Email is primarily used to send you information at this time. If you are sending us a document that was requested, please send in pdf format, instructions are available at the tab below.
On Mac
1. On your Mac, open the document you want to save as a PDF.
2. Choose File > Print.
3. Click the PDF button or click the down arrow to open the PDF pop-up menu, then choose Save as PDF.

5. Choose a name and location for the PDF file. Enter the information you want in the Title, Author, Subject, and Keywords fields. Later, you can search on the contents of those fields using Spotlight.
6. To protect your document with a password, click Security Options.
You can require passwords for opening the document, copying from the document, and printing the document.
On PC (Microsoft Word)
1. If the file was previously saved, select File > Save a Copy.
2. If the file is unsaved, select File > Save As.
3. Select Browse to choose the location on your computer where you want to save the file.
4. In the drop-down list, select PDF.
5. Select Save.
On PC (web based)
1. Select File > Print > Print (in PowerPoint you will select one of three formats).
2. In the dropdown menu under Printer , select Save as PDF and then select Save.
3. Saving as PDF option for printing.
4. Then in the Explorer menu that opens you can name the PDF and choose where to save it and then select Save.
5. This will generate a basic PDF that preserves the layout and format of your original document.
As our office is part of a Family Health Organization (FHO):
• We offer an After-Hours Clinic by physicians in our group, Monday to Thursday, starting at 5pm for same-day, urgent medical concerns. We are also available Saturday 9am-12pm. Call that day to book the appointment, as we are not accepting walk-ins at this time.

Please do not use a walk-in clinic if we are open, always call our office first for any of your medical needs, Monday to Friday 8:30AM-12:30PM and 2- 4PM as well as Saturday 9-11:30AM. We have access to your medical history and can provide more safe, effective care.
